<注意事項>

1. 為避免自行攜入造成之感染，若是由使用者自行入室，飼育員會先將入室動物充分消毒外箱後、放置動物房各區內，請使用者自行進入各區，再行入室動物。

2. 若有需要飼育員代為入室，請提前至少3日前申請此表格，以利飼育人員事前準備相關備品。

3. 第一箱收費100元，第二箱起收費降為50元。(例如: 若有兩箱動物，共收費150元；三箱動物，共收費200元，依此類推)

<Notes>

1. To avoid infection caused by introducing animals on user's own, the caretakers will thoroughly disinfect the outer carton (or box) of the animals before placing them in different areas of the animal room. Users are requested to enter each area  before introducing the animals by him/herself.

2. If needed, please apply at least 3 days in advance for this form so that the animal caretakers can prepare the necessary supplies.

3. The first box costs NT 100 dollars, and the price drops to NT 50 dollarts for each subsequent box. (For example, if there are two boxes of animals, the total cost is NT 150 dollars. Three boxes would cost NT 200 dollars, and so on.)
