<注意事項>

1. 為配合實驗動物正常作息時間以維護動物福利，動物房各區之日夜週期為上午7時至晚上7時止為開燈狀態，其餘時間皆為熄燈狀態。

2. 為兼顧使用者實驗方便性、避免工作人員無法提供周全服務，五流中的人員(使用者)被允許進出動物房之通行時間為週一至週五上午7時30分至晚上6時止，其餘時間及時段為禁止進出動物房(包含六日)、動物房內通行也會失效。

3. 若由於特殊實驗需求，需要在上述項目2.以外時段進出動物房各房間(包含移動)，需填寫此申請單，並由相關承辦人員暫時開立額外時段門禁，但必須於實驗日期結束後，終止並回歸正常的項目2.時段權限。

4. 基於上述特殊時段非屬正常工作時間，恕無工作人員提供服務，僅有攝影機可提供您協助。

1. To fit the normal lifestyle of experimental animal and maintain their welfare, the day and night cycle by each area of AScore is lights on from 7:00 AM to 7:00 PM, and lights off all the other times.

2. To ensure the convenience, user is allowed to in and out the animal room from 7:30 AM to 6:00 PM in the working days (Monday to Friday). In and out inside the animal room is deactivated all the other times (Saturday and Sunday are also deactivated), including the passing through access inside each room and area.

3. If any special experimental needs require additional times, please fill in this application form and AScore will temporarily activate access for you. Access must be deactivated to the normal hours in Section 2. if the experimental date has been finished.

4. Since the time period is off normal working hours, no staff will be available to provide services. Only monitors will be available to provide assistance.
